Terms and conditions of service

You are attending an appointment for sports massage therapy, which is a hands on skin process.

All clients are required to complete a client pre consultation & consent form online ready for their your first appointment.

If you are aged 16 years or under, please allow a parent/guardian to fill out the consultation form and they will need to be present for the appointment. Treatments will only be carried out if safe to do so. An assessment will be undertaken after reviewing the pre consultation form before every session to assess existing health conditions.

Contraindications​

Contraindications are listed below, and if any of these are present a further discussion will be required, as Louise may not be able to offer you a sports massage at this time.

  • Cancer (existing or in remission)

  • Acute injury (first 24-48 hours)

  • Fever or cold

  • Pregnancy

  • Suspected DVT

  • Hyperthermia

  • Infectious diseases or broken skin

  • Under the influence of alcohol/drugs

​Payments & Cancellations

  • Louise offers mobile sports massage and reduced appointments in Huntington. Please ensure you have space for the couch to be set up to ensure Louise can work unhindered.

  • In order to secure your booking, payment in full is required for all massage appointments.

  • Refunds are subject to a minimum of 24 hours notice cancellation prior to the scheduled appointment time. If less than 24 hours notice is given, the payment is non-refundable (unless the appointment slot can be offered to another client)

  • If an appointment is cancelled with less than 24 hours notice, your payment will not be refundable and cannot be transferred to a future appointment.

  • If you’re unable to attend your appointment for any reason, contact Louise as soon as possible.

  • If you’re late for your appointment, your session time will be reduced to ensure your appointment finishes on time.

Abuse & Inappropriate Behaviour Policy:

  • If at any point a client displays inappropriate behaviour (including verbal, physical or sexual conduct) the appointment will end immediately with no refunds given even if the full appointment time has not been completed.

  • Abuse of any kind will not be tolerated.

  • If a client is displaying inappropriate behaviour, the appointment will end.

  • Harassment of any kind is not tolerated.

  • Appropriate clothing is to be worn at all times during the massage. Loose clothing is preferred which will be protected by a towel.

Health, Safety and Consent

  • Before treatment, clients must provide accurate and up-to-date information about their health, injuries, medication, medical history and any relevant changes in condition.

  • This includes declaring any allergies or sensitivities to oils, creams, balms, tapes or other products that may be used during treatment.

  • By attending an appointment, you confirm that the information you provide is accurate to the best of your knowledge and that you consent to treatment within the therapist’s professional scope of practice.

  • If any information is withheld or inaccurate, treatment may be unsafe and liability may be limited as a result.

Right to Refuse or Stop Treatment

Treatment may be refused, postponed or stopped if:

  • it is not clinically appropriate

  • there is insufficient health information

  • the client appears unwell or unsuitable for treatment

  • the client is under the influence of alcohol or drugs

  • behaviour is inappropriate

  • the treatment environment is unsafe or unsuitable

This applies particularly to mobile appointments, where a safe, clean and appropriate working space must be available

Liability

Treatment is provided based on the information provided by the you and the therapist’s professional assessment at the time of the appointment.

Louise Sports Massage & Holistic Therapies accepts no liability for issues arising from undisclosed conditions, inaccurate information, or failure to follow advice given before, during or after treatment.

Privacy Policy

I collect personal and health information only where necessary to provide safe and effective treatment.

This may include your name, contact details, medical history, treatment notes and appointment records.

Your information is used to assess suitability for treatment, maintain accurate records and manage appointments.

Health information is treated as confidential and stored securely. Your data will not be shared with third parties unless required by law.

Records are retained in line with professional, insurance and legal requirements.

You have the right to request access to your data, request corrections, or ask questions about how your information is used.
